POSITION SUMMARY: As a key member of the Northpoint team, the Primary Counselor is responsible for providing counseling and case management services to clients at Northpoint. This position provides clinical assessment and direct care services for defined patient population. Services are provided to patients, families and/or groups and may be in residential, partial hospitalization, and outpatient treatment settings. This position provides treatment in one or more of the following areas: substance abuse, emotional and behavioral issues, and other mental disorders as outlined in the "Diagnostic and Statistical Manual of Mental Disorders" (DSM).


ESSENTIAL RESPONSIBILITIES AND DUTIES INCLUDE BUT ARE NOT LIMITED TO THE FOLLOWING:

· Provide assessment and direct care to individuals, families, and groups. Coordinate all aspects of patient care from admission to discharge, interacting with the multidisciplinary treatment team to provide quality patient care

· Provide education and orientation for patients, families and others to assigned treatment program to aid in therapeutic process. Collaborate with the patient in the development of a written treatment plan. Include patient and family involvement in assessment, treatment, education and discharge planning

· Monitor patients on an ongoing basis to continually assess needs and facilitate treatment and interventions

· Document clinical assessment, diagnosis, treatment plan, diagnostic summaries, and progress charting in a timely manner to assure comprehensive and quality patient care

· Actively participate as member of the multidisciplinary treatment team and maintain effective working relationships with employees and peers

· May provide clinical updates to representatives of managed care to assure coordination and reimbursement of treatments services. May conduct cognitive testing and document results

· Coordinate with community based providers and referral sources to ensure optimal levels of care post discharge and to maximize treatment outcomes

· Case staffing and individual case supervision

· Facilitate individual and group therapy

· Adhere to all company policies and procedures

· Maintain confidentiality in accordance with established policies and procedures and standards of care

· Other clinically appropriate services and special projects as assigned


QUALIFICATIONS/REQUIREMENTS FOR POSITION:

· Master’s Degree of Social Sciences preferred

· Maintain national and applicable state licensure requirements

· Minimum of 1 year experience in relevant field preferred

· Ability to work with a diverse population along with interpersonal skills and knowledge required in treating patients in area of assigned specialty

· An understanding of and adherence to the ethical standards of the respective license adopted by the governing board for licensure

· Adherence to Compliance Program Plan

REQUIRED CERTIFICATES/LICENSES/REGISTRATIONS:

· Must hold an active CADC, LPC, LCPC, LMSW, LCSW or LMFT

· Must maintain current nationally accredited CPR/First Aid/AED certification

· Must have a valid driver’s license and safe driving record

CONDITIONS OF EMPLOYMENT:

· Completion of a tuberculin screening during first week of employment

· Completion of a pre-employment drug screening, post-accident and upon reasonable suspicion of use

· Completion of all required paperwork prior to reporting to work, including acknowledgment of all Company policies

· Demonstrated computer literacy

· Completion of CPR/First Aid Training within first 90 days of employment

· Attendance at all mandatory staff development and training

· Completion of background check and professional references check

· Must be fully vaccinated for COVID-19
