Case Manager specializing in working with individuals with Substance Use Disorders. These services are in an outpatient setting and intended to assist the client in obtaining and utilizing community resources that assist them in their recovery.

JOB DUTIES
Create a safe and trusting space in order to build rapport with participants and their designated supports.
Complete various assessments, treatment plans, reviews, and progress notes in a timely manner.
Effectively collaborate and communicate with treatment providers and staff.
Maintain confidentiality of verbal and written communication.
Adhere to HSI policies and procedures as well as the NASW Code of Ethics.
Effectively navigate Electronic Medical Record systems (Athena. Training will be provided in house)
Complete 10 CEUs annually.

REQUIREMENTS
Previous training and experience working with co-occurring substance use and mental health disorders
Pass a criminal history background check and drug test
Positive, motivated, self-driven attitude
Case management skills such as assessing for needs, linking to resources, and coordinating care with external stakeholders
Bachelor Degree
PREFERRED SKILLS
Skills training in Motivational Interviewing
Computer literate and able to meet deadlines; familiarity with EHR systems
Familiarity with state funding including BPA and Medicaid
Bachelor Social Work License or CADC with Bachelors degree preferred

Pay starting at $20 - $24.50 per billable hour depending on experience and/or credentials.
$250.00 sign on bonus.
